Stevia extract contains mainly steviol glycosides, in which sugar molecules (the black in the above structure) are attached to steviol (the red part). Based on what it says in the paper below, the whole molecule is not absorbed, only the pieces after the sugars fall off in the intestine. From the other papers you posted, it sounds like it is the whole molecule that is active against Borrelia in vitro. If the whole molecule is not getting into the bloodstream in vivo, that would be a problem for activity.
