While the prior shots might have been a contributing factor I would place the ankle surgery he had as the prime suspect for the clotting. Clots after surgery are common, especially since people are told to avoid blood thinners before surgery to prevent excess bleeding. The clotting is likely a greater danger for many people than too much bleeding from surgery. Recently a well known female yoga guru/author around the same age as Hill died from a clot and embolism triggered by her ankle surgery:
